Apr 9, 2012 - Unprepared ATC. They should have had both planes go down the runway one after the other. After they got by the parked planes, they should have exited onto the taxi way and gone down to the end of the runway the normal way. Then they would have known where the other plane was at all times. Accident was caused by numerous human errors, which calm procedures could have rectified. Putting two planes on a runway for that length of time is asking for it.

And still without ATC clearance it ws a biggest mistake of Van Zantens to take off!! he represented an extreme authority figure to his subordinates intimidated them. His co-pilot and flight engineer had intuition that Pan Am is still on runway but due to lack of equal speaking rights in cabin they were afraid to say anything to their senior captain.
